348e114bbd block: move the ->devnode callback to struct block_device_operations
There really isn't any good reason to stash a method directly into
struct gendisk. Move it together with the other block device
operations.

8cd5b8fc00 block/diskstats: replace time_in_queue with sum of request times
Column "time_in_queue" in diskstats is supposed to show total waiting time
of all requests. I.e. value should be equal to the sum of times from other
columns. But this is not true, because column "time_in_queue" is counted
separately in jiffies rather than in nanoseconds as other times.

This patch removes redundant counter for "time_in_queue" and shows total
time of read, write, discard and flush requests.

ea18e0f0a6 block/diskstats: accumulate all per-cpu counters in one pass
Reading /proc/diskstats iterates over all cpus for summing each field.
It's faster to sum all fields in one pass.

Hammering /proc/diskstats with fio shows 2x performance improvement:

fio --name=test --numjobs=$JOBS --filename=/proc/diskstats \
    --size=1k --bs=1k --fallocate=none --create_on_open=1 \
    --time_based=1 --runtime=10 --invalidate=0 --group_report

	  JOBS=1	JOBS=10
Before: 7k iops	64k iops
After: 18k iops 120k iops

Also this way code is more compact:

add/remove: 1/0 grow/shrink: 0/2 up/down: 194/-1540 (-1346)
Function old new delta
part_stat_read_all - 194 +194
diskstats_show 1344 631 -713

2b8bd42361 block/diskstats: more accurate approximation of io_ticks for slow disks
Currently io_ticks is approximated by adding one at each start and end of
requests if jiffies counter has changed. This works perfectly for requests
shorter than a jiffy or if one of requests starts/ends at each jiffy.

If disk executes just one request at a time and they are longer than two
jiffies then only first and last jiffies will be accounted.

Fix is simple: at the end of request add up into io_ticks jiffies passed
since last update rather than just one jiffy.

Example: common HDD executes random read 4k requests around 12ms.

fio --name=test --filename=/dev/sdb --rw=randread --direct=1 --runtime=30 &
iostat -x 10 sdb

Note changes of iostat's "%util" 8,43% -> 99,99% before/after patch:

Before:

Device: rrqm/s wrqm/s r/s w/s rkB/s wkB/s avgrq-sz avgqu-sz await r_await w_await svctm %util
sdb 0,00 0,00 82,60 0,00 330,40 0,00 8,00 0,96 12,09 12,09 0,00 1,02 8,43

After:

Device: rrqm/s wrqm/s r/s w/s rkB/s wkB/s avgrq-sz avgqu-sz await r_await w_await svctm %util
sdb 0,00 0,00 82,50 0,00 330,00 0,00 8,00 1,00 12,10 12,10 0,00 12,12 99,99

Now io_ticks does not loose time between start and end of requests, but
for queue-depth > 1 some I/O time between adjacent starts might be lost.

For load estimation "%util" is not as useful as average queue length,
but it clearly shows how often disk queue is completely empty.

0226e9ead4 block: move the *_PARTITION enum out of genhd.h
The enum containing the *_PARTITION symbolic names is only relevant
for the partition parser. More specifically most values are MSDOS
partition table system indicators and thus should go straight into
msdos.c. One value is only used by the sun partition parser, and the
sun and sgi partition parsers use the same value as the x86 Linux
RAID indicator to also indicate RAID autodetection. Duplicate them
in sun.c and sgi.c given that the different partition types use
entirely different values otherwise.

9243c6f3e0 block: Document genhd capability flags
The kernel documentation includes a brief section about genhd
capabilities, but it turns out that the only documented
capability (GENHD_FL_MEDIA_CHANGE_NOTIFY) isn't used any more.

This patch removes that flag, and documents the rest, based on my
understanding of the current uses of these flags in the kernel. The
documentation is kept in the header file, alongside the declarations,
in the hope that it will be kept up-to-date in future; the kernel
documentation is changed to include the documentation generated from
the header file.

Because the ultimate goal is to provide some end-user
documentation (or end-administrator documentation), the comments are
perhaps more user-oriented than might be expected. Since the values
are shown to users in hexadecimal, the documentation lists them in
hexadecimal, and the constant declarations are adjusted to match.

Commit cd02cf1ace ("mm/hotplug: fix an imbalance with DEBUG_PAGEALLOC")
fixed memory hotplug with debug_pagealloc enabled, where onlining a page

generic_online_page() maps it first. This is somewhat wasteful, but
better than special casing page freeing fast paths.

it's actually enabled. One has to test debug_pagealloc_enabled() since
031bc5743f ("mm/debug-pagealloc: make debug-pagealloc boottime
configurable"), or alternatively debug_pagealloc_enabled_static() since
8e57f8acbb ("mm, debug_pagealloc: don't rely on static keys too early"),
but this is not done.

As a result, a s390 kernel with DEBUG_PAGEALLOC configured but not enabled
will crash:

Unable to handle kernel pointer dereference in virtual kernel address space
Failing address: 0000000000000000 TEID: 0000000000000483

Fix this by checking debug_pagealloc_enabled_static() before calling
kernel_map_pages(). Backports for kernel before 5.5 should use
debug_pagealloc_enabled() instead. Also add comments.

The first one is larger than usual, but the issue could not be solved simpler.
Also, it's a resend of the patch I submitted a few days ago, with a one line
fix on top of that: the size of the comment extensions was not taken into
account at reporting the full size of the set.

- Fix "INFO: rcu detected stall in hash_xxx" reports of syzbot
by introducing region locking and using workqueue instead of timer based
gc of timed out entries in hash types of sets in ipset.
- Fix the forceadd evaluation path - the bug was also uncovered by the syzbot.

a8e41f6033 icmp: allow icmpv6_ndo_send to work with CONFIG_IPV6=n
The icmpv6_send function has long had a static inline implementation
with an empty body for CONFIG_IPV6=n, so that code calling it doesn't
need to be ifdef'd. The new icmpv6_ndo_send function, which is intended
for drivers as a drop-in replacement with an identical function
signature, should follow the same pattern. Without this patch, drivers
that used to work with CONFIG_IPV6=n now result in a linker error.

c780e86dd4 blktrace: Protect q->blk_trace with RCU
KASAN is reporting that __blk_add_trace() has a use-after-free issue
when accessing q->blk_trace. Indeed the switching of block tracing (and
thus eventual freeing of q->blk_trace) is completely unsynchronized with
the currently running tracing and thus it can happen that the blk_trace
structure is being freed just while __blk_add_trace() works on it.
Protect accesses to q->blk_trace by RCU during tracing and make sure we
wait for the end of RCU grace period when shutting down tracing. Luckily
that is rare enough event that we can afford that. Note that postponing
the freeing of blk_trace to an RCU callback should better be avoided as
it could have unexpected user visible side-effects as debugfs files
would be still existing for a short while block tracing has been shut
down.

f3cc24942e Two fixes for the irq core code which are follow ups to the recent MSI
fixes:

  - The WARN_ON which was put into the MSI setaffinity callback for paranoia
reasons actually triggered via a callchain which escaped when all the
possible ways to reach that code were analyzed.

The proc/irq/$N/*affinity interfaces have a quirk which came in when
ALPHA moved to the generic interface: In case that the written affinity
mask does not contain any online CPU it calls into ALPHAs magic auto
    affinity setting code.

A few years later this mechanism was also made available to x86 for no
good reasons and in a way which circumvents all sanity checks for
interrupts which cannot have their affinity set from process context on
X86 due to the way the X86 interrupt delivery works.

It would be possible to make this work properly, but there is no point
in doing so. If the interrupt is not yet started then the affinity
setting has no effect and if it is started already then it is already
assigned to an online CPU so there is no point to randomly move it to
some other CPU. Just return EINVAL as the code has done before that
    change forever.

  - The new MSI quirk bit in the irq domain flags turned out to be already
occupied, which escaped the author and the reviewers because the already
in use bits were 0,6,2,3,4,5 listed in that order. That bit 6 was simply
overlooked because the ordering was straight forward linear
otherwise. So the new bit ended up being a duplicate. Fix it up by
switching the oddball 6 to the obvious 1.

f66ee0410b netfilter: ipset: Fix "INFO: rcu detected stall in hash_xxx" reports
In the case of huge hash:* types of sets, due to the single spinlock of
a set the processing of the whole set under spinlock protection could take
too long.

There were four places where the whole hash table of the set was processed
from bucket to bucket under holding the spinlock:

- During resizing a set, the original set was locked to exclude kernel side
add/del element operations (userspace add/del is excluded by the
nfnetlink mutex). The original set is actually just read during the
resize, so the spinlocking is replaced with rcu locking of regions.
However, thus there can be parallel kernel side add/del of entries.
In order not to loose those operations a backlog is added and replayed
  after the successful resize.
- Garbage collection of timed out entries was also protected by the spinlock.
In order not to lock too long, region locking is introduced and a single
region is processed in one gc go. Also, the simple timer based gc running
is replaced with a workqueue based solution. The internal book-keeping
(number of elements, size of extensions) is moved to region level due to
  the region locking.
- Adding elements: when the max number of the elements is reached, the gc
was called to evict the timed out entries. The new approach is that the gc
is called just for the matching region, assuming that if the region
(proportionally) seems to be full, then the whole set does. We could scan
the other regions to check every entry under rcu locking, but for huge
sets it'd mean a slowdown at adding elements.
- Listing the set header data: when the set was defined with timeout
support, the garbage collector was called to clean up timed out entries
to get the correct element numbers and set size values. Now the set is
scanned to check non-timed out entries, without actually calling the gc
  for the whole set.

Thanks to Florian Westphal for helping me to solve the SOFTIRQ-safe ->
SOFTIRQ-unsafe lock order issues during working on the patch.
